TRANSCRIPT POLICY AND PROCEDURES:
Transcripts are not available for pick-up. To ensure the accuracy of your transcript, please allow seven (7) business days for your transcript request to be processed. Once processed, your transcript will be mailed or faxed as indicated on your transcript order form. Individuals that are eighteen (18) or older MUST sign the release; parents are no longer allowed to sign for their daughter or son.
Health Records are available from 2010 to present. For more information you can contact the Student Records Department.
Please note that this department does not handle student transfers, you should contact the last school the student attended.
REQUESTS IN PERSON: Transcripts are not available on demand. In order to request a transcript or health record, you would need a picture ID, and a $5.00 money order for the processing fee. Office hours are from 9:00am-1:00pm, Monday-Friday at James Williams Learning Center, 401 Katherine Dunham, East St. Louis, IL 62201.
BY MAIL: Individuals may send a written request, a copy of their photo ID, and a $5.00 money order for processing fee. The request can be mailed to East St. Louis School District 189, Attn: Student Records, 1005 State Street, East St. Louis, IL 62201. The information needed is listed below:
- First Name, Middle Name, Last Name, (if married please include Maiden Name)
- Date of Birth
- Last four (4) digits of your Social Security Number
- School Name
- Graduating Year or (Last school you attended)
- Address to mail transcript
- To fax transcript, include fax number and contact person
- Phone Number

East St. Louis School District 189 has partnered with Parchment to send your official transcript quickly and securely to outside institutions.
Transcript requests are free during your senior year. On August 1st, following graduation, there will be a fee of $5.00 associated with sending your transcripts.
All orders are processed during normal school business hours.
Third party requests must also be made through the Parchment system.
If you are applying to a college that does not accept Parchment, please stop by Dr. Mateen’s office or the front office to request transcripts.
LAW OFFICES, COURTS: Third Party Agencies, Probation & Corrections, and all other Agencies, must pay a $10.00 processing fee for individual records with an appropriate consent form signed by the former student. The request must include the Individuals name at the time of graduation, school and year of graduation, and date of birth. All requests will be returned by postal mail or faxed upon your request.
